Toronto (April 3, 2017) — NexJ Health Inc. (“NexJ Health”), a provider of patient engagement solutions for chronic disease management, today announced that they have been selected for an innovative program through the Health Technology Fund to enhance onsite delivery support practices for Young Adults struggling with mental health issues.

This two-year project involves two mental health organizations serving young adults, Reconnect Community Health Services with Stella’s Place and St. Joseph’s Healthcare Hamilton – Youth Wellness Centre.  The providers will use NexJ Connected Wellness for virtual care and online support for up to 1000 Young Adults over a 22-month evaluation period to augment their current onsite delivery practices. NexJ Connected Wellness is expected to increase efficiency and enhance the capacity of these organizations to support Young Adults in Toronto and Hamilton to achieve enhanced self-management and health outcomes while delivering a shift to community-based care.

To support the Project and enable future broader Ontario adoption, an extended Health Innovation Team including CAMH, UHN, The Royal’s Institute of Mental Health Research, CHEO, and St. Michael’s Hospital are contributing time and advice.

The needs of Young Adults aged 16 to 29 with mental illness are not currently being met by traditional mental health services. Treatment is fragmented across the continuum of care and is not responsive to the developmental needs of the Young Adult age group. Use of expensive acute hospital-based care is increasing, and a shift to a community care model is needed. As young adults transition to adulthood, there is limited opportunity to engage in effective, integrated, community-based wellness management.  They are seeking opportunities to access information and services online, and to manage their health.
