Toronto (October 22, 2018) – NexJ Health Inc. (“NexJ Health”), a provider of patient-facing population health management solutions, was selected by the University Health Network (UHN) to support the ACCELERATION 2.0 program, delivering a behaviour change eHealth solution to support the prevention of chronic diseases, specifically cardiovascular and chronic respiratory diseases, cancer, and type 2 diabetes.

Building on the successes of a previously demonstrated model of care, ACCELERATION 2.0 is a 12-week behavioural change intervention that provides best practice strategies for the control and management of modifiable risk factors. The platform will assess the health and determinants of wellbeing of each user by having them complete a series of automated assessments and integrate electronic devices and activity trackers for objective measures. Under the review of a certified health coach, results and information extrapolated during the onboarding and intervention phase will generate more personalized care plans which will be reviewed during health coach initiated contacts (six in total). The delivery of counselling sessions will be multi-modal and align with participant needs and preferences.

It is well-documented that chronic diseases are both common and costly and that positive modification of health behaviours is the key to prevention. The choice of adopting positive or negative health behaviours is one that Canadians make every day.  The goal of ACCELERATION 2.0 is to make the healthy option the easiest and most attractive one through a tried and true technology platform designed by NexJ Health. With an overarching mission of building a healthier Canada, we plan to help participants “choose health – every day”.

Dr. Paul Oh, Medical Director of the Cardiovascular Prevention and Rehabilitation Program at Toronto Rehab, says, “This program will reduce the burden of chronic diseases by targeting common modifiable risk factors and delivering an evidence-based behaviour change program through NexJ Connected Wellness. The reach of the ACCELERATION program has been really gratifying – we are so excited about the scalability now that it will be supported by a digital platform.”

Gary Baksi, Executive Vice President and General Manager at NexJ Health, says, “NexJ Health is excited to collaborate with UHN and Medavie to deliver a virtual care program to engage and activate participants to make lasting lifestyle changes to reduce the onset of chronic disease. This project is a strategic and natural extension of NexJ Health’s recent collaboration with the Heart and Stroke Foundation of Canada, preventing hypertension in Canadians through health coaching for lifestyle changes.”

NexJ Health, UHN and Medavie Health Foundation will work together to deliver this intervention with initial plans to scale across Ontario, Quebec, British Columbia, and Atlantic Canada.
