University Health Network Selects NexJ Health for Chronic Disease Prevention Program
Toronto (October 22, 2018) – NexJ Health Inc. (“NexJ Health”), a provider of patient-facing population health management solutions, was selected by the University Health Network (UHN) to support the ACCELERATION 2.0 program, delivering a behaviour change eHealth solution to support the prevention of chronic diseases, specifically cardiovascular and chronic respiratory diseases, cancer, and type 2 diabetes.
Building on the successes of a previously demonstrated model of care, ACCELERATION 2.0 is a 12-week behavioural change intervention that provides best practice strategies for the control and management of modifiable risk factors. The platform will assess the health and determinants of wellbeing of each user by having them complete a series of automated assessments and integrate electronic devices and activity trackers for objective measures. Under the review of a certified health coach, results and information extrapolated during the onboarding and intervention phase will generate more personalized care plans which will be reviewed during health coach initiated contacts (six in total). The delivery of counselling sessions will be multi-modal and align with participant needs and preferences.
It is well-documented that chronic diseases are both common and costly and that positive modification of health behaviours is the key to prevention. The choice of adopting positive or negative health behaviours is one that Canadians make every day. The goal of ACCELERATION 2.0 is to make the healthy option the easiest and most attractive one through a tried and true technology platform designed by NexJ Health. NexJ Health, UHN and Medavie Health Foundation will work together to deliver this intervention with initial plans to scale across Ontario, Quebec, British Columbia, and Atlantic Canada.
