Habit Lifestyle Medicine is using NexJ Connected Wellness to deliver the Optimal Health Program which enables their full spectrum of lifestyle medicine to participants digitally, to decrease barriers of accessibility and distance. Optimal Health begins with a comprehensive assessment, which includes a complete review of medical history and health risk factors. The Habit Lifestyle Medicine team, including a Nurse Practitioner, Nutritionist, Sleep Coach, Personal Fitness Trainer, and Health Coach, carry out the lifestyle and nutritional assessment, fitness testing, and health coaching. The use of NexJ Connected Wellness in the Optimal Health Program improves the participant’s ability to share information, coordinate their care across their Habit care team, and receive personalized educational content to learn more about their conditions, medications, and treatments. Participants also track their behaviors, biometrics, and psychometrics to monitor their health and wellness. 

The increase in chronic disease prevalence and associated health care spending globally is unsustainable. Chronic conditions such as hypertension, heart disease, stroke, type 2 diabetes, obesity, and cancer are among the most common and costly of all health conditions—but they are also largely preventable.1 Lifestyle medicine focuses on the health behaviors that give rise to these diseases.
