Toronto, Ontario – October 1, 2018 – NexJ Health Inc. (“NexJ”), a provider of patient-facing population health management solutions, today announces the awarded contract for the Call & Check program in the Channel Island of Jersey, supporting socially isolated, frail elders stay at home longer, leveraging a virtual care platform for upstream population health management.

Approximately 1% of the Western population is 65 or older, lives alone[1], and has used homecare in the past 12 months[2]. This population demonstrates a marked need for services that will provide both social contact to reduce isolation, and improved access to care.

Call & Check is an award-winning and innovative service that has been developed and piloted by Jersey Post. It offers a regular visit to members of the community who could benefit from some additional help and support, leveraging a trusted community resource and infrastructure: the postal worker. The program enables postal workers on their usual postal rounds to check on the well-being of vulnerable members of society and to connect them with supportive community services, all for the cost of a registered letter.

Through Call & Check, postal workers check in on these individuals twice a week, ask them a short survey of questions around health, well-being, social isolation, and other locally relevant concerns, and capture the answers through the platform on their handheld devices. The software runs seamlessly on the Jersey Post postal worker’s handheld devices, allowing them to capture important information at the doorstep that is shared immediately with the person’s personal circle of care. Social services and community support are triaged as needed, enabling efficient coordination of care, and increased access to care.

The 12-month project is a collaboration between Jersey Post, HomeCall, and NexJ Health. NexJ Connected Wellness will be provisioned and hosted in the UK, complying with General Data Protection Regulation (GDPR) requirements for individuals within the European Union.
