NexJ Health Coach to be used to engage patients and to assist in chronic and pre-chronic disease management services

Toronto, ON (April 25, 2013) — NexJ Systems Inc., (TSX: NXJ) a provider of cloud-based software, delivering enterprise solutions for the financial services, insurance, and healthcare industries today announced the Leduc Beaumont Devon Primary Care Network (LBDPCN), a joint partnership between Alberta Health Services and a group of 51 family physicians in 11 clinics, has selected NexJ Connected Wellness to engage patients and assist in the management of chronic and pre-chronic disease.

LBDPCN has licensed NexJ Connected Wellness on a subscription basis for use in their Self Manage to Improve your Life Everyday (S.M.I.L.E.) healthcare program. The S.M.I.L.E. healthcare program brings together a team of healthcare professionals that help people living with various chronic conditions such as diabetes, pre-diabetes, high blood pressure, high cholesterol and obesity. The team, which includes registered nurses and dietitians, a registered pharmacist and psychologist and an exercise specialist, works with patients to help them learn strategies to improve their ability to manage their health.

Patients will use NexJ Health Coach to capture biometrics such as weight, blood glucose and blood pressure. They enter this data using a smartphone or tablet, and can track their daily activities such as exercise, food intake, as well as subjective information such as how they feel.
The S.M.I.L.E. team will review the information and provide feedback through secure messaging and jointly review the accumulated data with patients. Helping patients understand the relationship between their behaviors, their biometric measurements and how they feel, delivers the motivation and encouragement individuals need to sustain the behavior changes necessary to improve their wellness.
