(“NexJ Health”), a provider of AI-enabled digital health and coaching solutions, today announced the launch of an integration with UC San Diego Health built on FHIR (Fast Healthcare Interoperability Resources), a global standard for exchanging electronic health information. The integration enables patient reported outcomes collected through NexJ Connected Wellness to be securely shared with Epic.

This integration enables UC San Diego Health to collect, operationalize, and get reimbursed for patient reported outcomes from the Patient Health Questionnaire-9 (PHQ-9) captured outside traditional visits—while ensuring those data are available within Epic, its clinical system of record.

Extending clinical capacity beyond the visit

As health systems continue to adopt measurement-based care and value-based models, the demand for consistent patient reported data increases. Collecting this information during in-person visits alone is impractical given clinic capacity constraints, and rising documentation burdens.

NexJ Health acts as an extension of the clinic, providing targeted health coaching support for patients identified by primary care providers as experiencing mild to moderate depression symptoms. Between medical appointments, coaches are available to help these patients adopt healthier practices related to diet, exercise, sleep, and stress management, resulting in measurable improvements in depression symptoms as tracked by PHQ-9 scores within NexJ Connected Wellness. By integrating these patient-reported outcomes directly into Epic, UC San Diego Health providers can view and interact with these mental health improvements, broadening clinical impact without adding the need for additional visits. Furthermore, this integration enables UC San Diego Health to access reimbursement for mood improvements achieved through the program, reinforcing both operational efficiency and financial sustainability.

PHQ-9 responses are the first patient reported outcome supported through this integration and serve as an initial use case in a broader strategy designed to support additional PROs across all programs.
